All necessary enrollment forms must be completed before children are enrolled. 

The following forms must be completed by parents or legal guardian:

​3) Child's Emergency Medical Authorization (Click here to download)

​4) Commonwealth of Virginia School Entrance Health Form (Click here to download)

​Parents or legal guardian must also provide the following information:

1) Child(ren) original birth certificate or hospital's birth letter

2) Health and physical records with updated immunizations signed by the child's physician

3) Child(ren) original social security card

4) Custodial paperwork if applicable

5) Complete information for two emergency contacts

6) Complete home and employer information 

7) Child's physician complete information
